For More Information

To schedule a pediatric neurology appointment, please call 847.570.2577 or visit NorthShoreConnect. Clinic appointments may require a referral from your child’s pediatrician, and your insurance plan may require paperwork or other authorization for a visit with one of our specialists. Please contact your child’s physician and your insurance provider for more information.

If you need to refill a prescription, plan ahead as it may take up to 48 hours.

In addition, please have your primary care physician send relevant laboratory results and notes. If another pediatric neurologist has seen your child previously, a copy of the consultation notes, lab results and prior evaluations, including disc and written copies of EEGS, MRIs and CT scans, are also requested for review. You can immediately access your NorthShore medical records and view test results as well as communicate with your child’s care team through NorthShoreConnect.

Any required forms can be faxed to 847.733.5424.
