Community Health Needs Assessment (CHNA)

NorthShore is required to conduct a CHNA every three years to determine the needs of the community and to ensure that its community benefits programs are in alignment with the health needs of the communities it serves.

Your participation is a valuable asset and an important contribution as we gather information on health disparities and challenges of our service area as part of our system-wide assessment.
