Information For Your Vaccination Appointment

  • Parents or legal guardians of minors (i.e. all patients under 18 years old) must physically accompany the minor to the vaccination appointment and provide consents for care for the vaccine to be administered.  For more information on the vaccine and what to know before your appointment, visit our COVID-19 Vaccination Program page.
  • For those getting vaccinated for the first time, NorthShore is administering the Pfizer vaccine.  For those requiring an in-brand booster, Moderna is available.
  • Please bring your ID and insurance card (if available). If you are getting a second or third dose, please bring your CDC vaccination card.

Swedish Hospital is accepting walk-in's 12 year olds and up*.

*Due to overwhelming demand for COVID-19 vaccinations for children between the ages of 5 and 11, Swedish Hospital ONLY is temporarily pausing COVID-19 vaccinations for this age group while we await new vaccine supplies. Swedish Hospital will still administer scheduled second doses for this age group and remain open for first doses for everyone aged 12 and older and for all those eligible for boosters.
