NorthShore COVID-19 Vaccination Clinics

It’s fast and easy to get you and your loved ones (anyone as young as 5 years old) COVID-19 vaccinations. Schedule an appointment via NorthShoreConnect, or call 847.982.5000.

Information For Your Vaccination Appointment

  • Parents or legal guardians of minors (i.e. all patients under 18 years old) must physically accompany the minor to the vaccination appointment and provide consents for care for the vaccine to be administered.  For more information on the vaccine and what to know before your appointment, visit our COVID-19 Vaccination Program page.
  • For those getting vaccinated for the first time, NorthShore is administering the Pfizer vaccine.  For those requiring an in-brand booster, Moderna is available.
  • Please bring your ID and insurance card (if available). If you are getting a second or third dose, please bring your CDC vaccination card.
