Effective September, 2017, as a vitally important patient safety initiative, all NorthShore University HealthSystem employees, medical staff, sub-contracted vendors, students, volunteers and trainees will be required to receive an annual influenza vaccination.
Need a Flu Shot?
Employees can receive the vaccination by appointment only and provided FREE of charge. To save time, please complete the consent form and bring it with you to your appointment.
Got your Flu Shot outside of NorthShore?
To provide evidence of administration outside of Northshore (Walgreens, your doctor’s office etc…) please provide the following:
- A document that contains evidence of administration: your name, date shot was administered, and who administered it.
- This completed verification form.
- Fax both your evidence of administration and verification form using this fax coversheet.
