Cardiac Rehabilitation is a program of exercise, education and counseling designed to assist individuals in their recovery from heart problems.
Phase II -Early Out-Patient Reconditioning Program
For patients who have recently been hospitalized (usually within 3-6 months). Heart rate and rhythm are monitored during individually tailored exercise sessions. Structured cardiac risk factor education and counseling is included as a part of the program. Call for an appointment.
Phase III -Maintenance Program
For individuals who have completed the Phase II program or whose hospitalization was more than 6 months ago or others as determined by their physician. Participants of the Phase III program are considered to be more independent with the exercise, however cardiac rehabilitation staff are available to assist with exercise progressions and to monitor for symptoms and proper exercise response. Call for an appointment.
