Heart Failure Program
Who We Are
The Heart Failure Program of NorthShore University HealthSystem is committed to assisting patients with heart failure to achieve and maintain an optimal level of health. Our program provides you and your family continuity of care through your hospital stay and beyond.
Our team of Advanced Practice Nurses (APNs) will collaborate with your Cardiologist while you are hospitalized to assist in your plan of care. Patients with heart failure will benefit from comprehensive care including the application of the most recent heart failure clinical guidelines. We also provide individualized treatment, education, lifestyle modification and diet recommendations.
Short term follow up care may be provided to confirm that you and your family understand your care and needs after discharge. Post hospital symptom monitoring will help keep you at home and feeling your best.
Within this program, patients are screened and evaluated for the use of the latest technological devices including cardiac resynchronization therapy and ICD's. Patients can be referred for treatment options such as non surgical cardiac procedures for heart failure like valvuloplasty and percutaneous mitral valve repair. Some patients may be eligible to participate in research & clinical trials.
We will work with other members of the health care team to put you in touch with the resources necessary to allow you to live your life to the fullest.
What to Do
If you have congestive heart failure, ask your Cardiologist if you would benefit from the services of the program.
