Sports Health and Performance Program Packages
The NorthShore Sports Health and Performance Program offers a variety of options to help you achieve your personal athletic, health and performance goals through a comprehensive and highly individualized approach to your training. All of our packages include a visit with a Primary Care Sports Medicine physician.
Executive Sports Health and Performance Package
Our most comprehensive package helps you successfully achieve your health and performance goals. Athletes receive written reports documenting ongoing results and recommendations that can be shared with your training team. This package consists of: an initial Sports Health and Performance assessment, three physician follow-up visits, three follow-up Functional Movement Screen (FMS) evaluations, three follow-up Bod Pod tests, a dietitian initial evaluation and four dietitian follow-up visits.
Elite Performance Package
This two-hour initial Sports Health and Performance assessment includes a complete sports physical and an individualized health and performance plan, including recommendations for follow-up monitoring and additional testing. Along with a Bod Pod test and FMS evaluation, athletes receive a written report with results and recommendations. Care is coordinated with your personal trainer.
Advanced Sports Physical Package
Geared toward collegiate and high school athletes, this 60-minute package includes a comprehensive sports physical with extended nutrition, hydration and training counseling, an FMS evaluation and a Bod Pod test. Athletes receive a written report with results and recommendations.
Sports Physical Package
For high school and grade school athletes, this 45-minute package includes a comprehensive sports physical with extended nutrition, hydration and training counseling, and an FMS evaluation. Athletes receive a written report with results and recommendations.
