NorthShore University HealthSystem (NorthShore) Home and Hospice Services is an integral part of the NorthShore organization, providing complete continuity of care to patients in their own homes. A hospital-based, non-profit agency, Home and Hospice Services is proud of its 100-year heritage and tradition of excellence. Our clinical staff works with patients, families and physicians to offer the highest level of personalized care based on individual needs. We follow the same protocols and standards of practice used in our award-winning hospitals and our clinical staff members are available to home-bound patients 24 hours a day, seven days a week.

We offer a full compliment of home health services from our experienced clinical staff representing a wide array of medical specialties and infusion therapies to home medical equipment needs and hospice care. NorthShore Home and Hospice Services is Medicare certified and CHAP accredited (Community Health Accreditation Program).  Many of our physician-prescribed services are covered by Medicare and other insurances.

As part of NorthShore, we have full access to our electronic medical records so clinical staff are immediately up-to-speed with patients’ medical conditions and history. Our clinical staff work from physician orders, and respond with regular reports back to referring doctors, keeping primary care physicians up to date on their patients’ condition and progress. All labs drawn by our home health services are recorded in the patient’s electronic medical record.
