11/3/2009 - Diabetic patients now have another tool to help manage their diabetes through the advanced capabilities of NorthShoreConnect. 

NorthShore University HealthSystem’s (NorthShore) free, secure web portal service is already helping close to 100,000 people manage their medical care, and now with the addition of a diabetes management tool, NorthShoreConnect offers even greater benefits for patients challenged with a chronic condition.

The diabetes management tools now included in NorthShoreConnect enable and encourage patients to take a more active role in the care and management of their disease. 

Patient-entered flowsheets allow diabetic patients to record their glucose and/or insulin levels at the recommended intervals in their NorthShoreConnect account. The flowsheets are automatically submitted to the patient’s physician for review, allowing physicians to regularly monitor a patient’s progress. Abnormal results are immediately flagged for physician evaluation and recommendation. 

Diabetic patients and their physicians also have 24/7 access to their current medications. Diabetic patients have a Diabetes Preventative Care Summary in their account which includes their current prescriptions, most recent lab results, the dates of their last diabetes related labs and reminders of when they are due for their next lab tests. 

NorthShoreConnect and its diabetes management tools are designed to help patients take a more pro-active role in their health, leveraging technology and convenience to their personal advantage.
