NorthShore University HealthSystem (NorthShore) offers a variety of life insurance coverage to protect your family.

Basic Term Life

NorthShore provides coverage equal to 1 or 2 times annual base salary, including Accidental Death and Dismemberment coverage for loss of life or limb.

Supplemental Life

Purchase additional life insurance coverage equal to 1 or 2 times annual base salary, up to a maximum of $1.5 million when combined with basic term life.

Dependent Term Life

Purchase coverage for spouses and children, up to a maximum of $100,000 for spouses and $10,000 for children.

Voluntary Accidental Death & Dismemberment

Choose individual or family coverage, up to a maximum of 10 times salary or $500,000 (whichever is less).

Travel Accident Coverage

NorthShore provides coverage for loss of life or limb while traveling on company business, up to a maximum dollar limit.
