Chirin Dankha is the pharmacy manager of the NorthShore University HealthSystem Community/Retail Pharmacy located in Lake Forest, IL. She is responsible for the day-to-day operations of a retail pharmacy, which in addition to dispensing prescriptions, include providing customer service, clinical services (immunizations, MTM, etc), inventory/financial management, and personnel management. She is one of the preceptors for the CPRP Direct Patient Care rotation. Chirin is also a preceptor for the Rosalind Franklin University of Medicine and Science (RFUMS) pharmacy students. She received her PharmD degree from Midwestern University-Chicago College of Pharmacy in 2004. She has extensive retail pharmacy staffing experience and has also worked in the managed care setting where she helped develop, implement and enhance clinical services such as disease state programs and MTM programs for PBM clients.
Sanford Foreman is the pharmacy manager at the Northbrook pharmacy. He received his Bachelor of Science in Pharmacy degree from the University of Illinois at Chicago College of Pharmacy. He has extensive community pharmacy experience as he owned and managed his own independent pharmacies for 20 years. Sandy is a member of the Pharmacy and Therapeutics Committee for outpatient pharmacies and is the liaison for the employee prescription health plan.
Mita Gupta is a clinical pharmacist at the anticoagulation clinic and inpatient pharmacy in Glenbrook Hospital. She received her Pharm.D. degree from the University of Illinois College of Pharmacy and completed a pharmacy practice residency with an emphasis in ambulatory care at the Jesse Brown VA Medical Center in Chicago. Her interests include cardiology, endocrinology, anticoagulation, and disease state management programs.
Ksenia Hankewych is the Residency Coordinator for the NorthShore CPRP and a Preceptor (Lecturer) at RFUMS College of Pharmacy. She oversees the pharmacy-managed anticoagulation service at Glenbrook Hospital and also works in the NorthShore community pharmacies. In addition to managing the anticoagulation service at NorthShore, Ksenia coordinates the community pharmacy immunization initiatives and community pharmacy patient-centered programs. She received her Doctor of Pharmacy degree from the University of Illinois College of Pharmacy in 2004 and completed a pharmacy practice residency with an emphasis in ambulatory care at the Jesse Brown VA Medical Center in Chicago in 2005. Ksenia was a member of the Rho Chi National Pharmaceutical Honor Society.
Tina Kolovos-Zook is the Community Pharmacy Residency Program Director of the NorthShore CPRP in addition to being the pharmacy manager at the Libertyville Sunset Foods Pharmacy location and the Wellness Shoppe in the Glenview Park Center. Tina is a Clinical Assistant Professor at RFUMS College of Pharmacy. She coordinates many of NorthShore's retail programs and services, including the immunization program, physician seminars, and various health screenings. In 2006, Tina started the CPRP at NorthShore. She also develops and maintains pharmacy operations and policies and procedures for the outpatient pharmacy department. She is responsible for interviewing, hiring and orientation of all new pharmacy personnel. She received her Doctor of Pharmacy degree from the University of Illinois College of Pharmacy in 1998.
Diane Nitzki-George is a clinical pharmacist at the anticoagulation clinic in Glenbrook Hospital and home infusion pharmacy at Evanston Hospital. She received her BS in chemistry from Northern Illinois University, BS in pharmacy from Creighton University in Omaha, NE, and MBA from Keller School of Business, and her PharmD degree from the University of Florida. Diane is also a medical writer and has several publications, including co-authorship in an ASHP book chapter. Diane is the editor for the monthly pharmacy newsletter and department web publisher. She is immunization and MTM certified through APhA. Her interests include anticoagulation, clinical nutrition, cardiology, and parenteral drug therapy. Diane is an Assistant Clinical Professor at the University of Florida Working Professional PharmD Degree program of the College of Pharmacy.
Shilpa Patel is the pharmacy manager at the Glenbrook Hospital Outpatient Pharmacy and a Preceptor (Lecturer) at RFUMS College of Pharmacy. Shilpa has worked as a pharmacy manager at NorthShore for over 6 years. Shilpa is the Outpatient Pharmacy Liaison to the NorthShore's corporate Medication Safety Officer. She is responsible for investigating and reviewing any outpatient pharmacy medication errors/events and collaborating with administration to remove sources of errors from the outpatient pharmacies. Shilpa was instrumental in establishing means for electronic billing of Medicare Part B for oral chemotherapy agents which eventually led to the establishment of an outpatient oncology pharmacy at NorthShore. She received her Doctor of Pharmacy Degree from the University of Illinois at Chicago College of Pharmacy in 1995. Shilpa will be responsible for precepting, coordinating, and mentoring patient care activities for the CPRP.
Kathryn Schiavo is a Clinical Oncology Pharmacist at the Kellogg Cancer Center. In addition to dispensing medications and providing clinical services, she coordinates all pharmacy services for the Point of Care Outpatient Pharmacies at the Evanston, Glenbrook, and Highland Park Hospitals and assists with performance improvement projects. Kathryn is one of the preceptors for the PGY2 oncology residency and pharmacy student summer internship program. She is also a Lecturer at the Rosalind Franklin University College of Pharmacy. She received her PharmD from Drake University College of Pharmacy and Health Sciences in Des Moines, IA and completed a PGY1 Community Pharmacy Residency at NorthShore University HealthSystem.
