Care Team
The NorthShore University HealthSystem Hospice and Palliative Care Team
NorthShore’s Hospice program revolves around cohesive, interdisciplinary teams including physicians, registered nurses with extensive experience in symptom and pain management, licensed clinical social workers, clinical pharmacists, certified nursing assistants, chaplains, volunteers and bereavement experts all committed to working together to ensure patients and families have the best possible experience in a most challenging time.
Our highly-skilled staff has special training and an interest in continuing education in the latest techniques and therapies to address patient and family physical and psychosocial symptoms and conditions. Weekly team meetings help identify potential problems and successful interventions designed to maintain a peaceful and comfortable environment for the patient and their loved ones.
Our hospice team includes expert physicians who are board-certified in hospice and palliative medicine. NorthShore’s hospice nurses are Magnet recognized, a prestigious designation shared by only 6 percent of healthcare organizations in the country.
Years and years of experience, and a low turnover rate reflect the hospice team’s dedication and expertise in end-of-life care.
NorthShore’s hospice program is:
- Accredited by the Community Health Accreditation Program (CHAP)
- Fully licensed in Illinois
- Medicare certified
I have been a nurse for more than 20 years and I love my job; this is why I went to nursing school. It’s a privilege to be able to care for patients entering the last stage of life and to be let into their home and their lives. I think when you invite the hospice team into your home we become a part of your family, but you also become a part of our NorthShore hospice family.
